On-line Booking Form
Please fill out as much of the form as you can. There are several required fields that are marked with a *. After submitting the Booking form
we will hold your date for 6 days awaiting payment. If no down payment has be received by 6 days your date will no longer be locked in and
will become available for a different booking. Once submitted you will be redirected to a new page with further information about payments
and contacting JHRJ Productions. If we have already had a consultation and or already worked out a payment plan please just write the
details in the additional information section. Thank You.   
Payments: If you selected to send your payment in by mail please mail to JHRJ Productions 702 SW 48th st ANKENY, IA 50023 and
make the checks payable to JHRJ Productions. If you selected to pay by Credit Card or PayPal an email will be sent to EMAIL 1 that you
provided above. The E-mail will have a Payment button linked to a secure PayPal page that will walk you though the payment process. You
do not need a PayPal account to use this option. Just click on the pay with credit card option to pay with VISA, MasterCard, American
Express, Discover, or a Check Card With VISA or MasterCard logo. A copy of the invoice will be sent to you in a separate email in order to
keep our payment process as secure as possible. The invoice will be in Adobe PDF formate and will be sent at the same time or before the
Payment email.
